О ТРУДНОСТЯХ ДИФФЕРЕНЦИАЛЬНОЙ ДИАГНОСТИКИ МЕЖДУ ПЕРВИЧНЫМ ДЕПЕРСОНАЛИЗАЦИОННО-ДЕРЕАЛИЗАЦИОННЫМ РАССТРОЙСТВОМ И ШИЗОТИПИЧЕСКИМ РАССТРОЙСТВОМ
Аннотация
Цель исследования. В данной работе мы ставили своей целью показать (как с помощью обзора данных, опубликованных в доступной современной научной литературе, так и с помощью представления читателю двух клинических случаев из нашей собственной практики), в чём могут заключаться нередко возникающие у практикующих клиницистов объективные трудности в дифференциальной диагностике между первичным деперсонализационно-дереализационным расстройством (ДПР, код 6B66 по МКБ-11, F48.1 по МКБ-10) и шизотипическим расстройством (ШТР, код 6A22 по МКБ-11, F21 по МКБ-10), каковы могут быть причины этих затруднений, и как же всё-таки различать эти расстройства в реальной, повседневной клинической практике.
Методология проведения работы. Вначале мы обратились к хорошо известным специалистам монографиям по ДПР, в частности, к книге Дафны Симеон «Feeling Unreal: Depersonalization Disorder and the Loss of the Self» и книге Маурицио Сиерры «Depersonalization: A New Look at a Neglected Syndrome» и к аналогичным по полноте и масштабу охвата монографиям по ШТР, в частности, к книге Оливера Мэйсона и Гордона Клэриджа «Schizotypy: New dimensions» и книге Адриана Райне «Schizotypal Personality», а также к релевантным источникам, использованным этими авторами в соответствующих главах своих книг.
Затем мы провели поиск других релевантных статей по интересующей нас тематике, с использованием таких ключевых слов, как depersonalization disorder, schizotypal disorder, differential diagnosis, в международных научных базах данных PubMed, Google Scholar, Web of Science, Elsevier ScienceDirect, Elsevier ClinicalKey.
Найденные нами литературные данные были затем проанализированы, обобщены и представлены в настоящей статье, вместе с детальным описанием и клинико-психопатологическим анализом двух клинических случаев из нашей собственной практики.
Результаты. Полученные нами в ходе составления настоящего литературного обзора данные, а также наш собственный клинический опыт, представленный в этой статье в виде описания двух случаев из нашей практики, на наш взгляд, убедительно свидетельствуют о том, что дифференциальная диагностика между первичным ДПР и ШТР порой бывает объективно затруднительной. Тем не менее, такая дифференциальная диагностика – не только возможна, но и необходима.
В психиатрии правильный диагноз имеет огромное практическое значение для последующего выбора правильной стратегии и тактики лечения психических расстройств – как психофармакологического, так и психотерапевтического.
Наши данные убедительно свидетельствуют также о том, что, к сожалению, среди специалистов в России и других постсоветских странах чрезвычайно распространены гипердиагностика ШТР в тех случаях, когда в действительности у пациента имеет место быть первичное ДПР, или же синдром деперсонализации-дереализации (ДП/ДР) в рамках какого-то другого психического расстройства (обычно – из тревожного или аффективного спектра), а также недостаточное понимание самой сути синдрома ДП/ДР, его типичной клинической симптоматики и феноменологии, недооценка его важности в общей клинической картине того или иного психического расстройства, недооценка важности специфических мер по его устранению у конкретного пациента, недостаточное умение диагностировать и лечить этот синдром. В свете этого, мы полагаем данную статью особенно актуальной для психиатров России и стран СНГ.
Область применения результатов. На наш взгляд, полученные нами в ходе составления настоящего обзора результаты заслуживают самого широкого применения в психиатрии.
