НЕСПЕЦИФИЧЕСКАЯ БОЛЬ В ПОЯСНИЦЕ: ДИАГНОСТИКА И ТЕРАПИЯ С ПОЗИЦИЙ ДОКАЗАТЕЛЬНОЙ МЕДИЦИНЫ (ОБЗОР ЛИТЕРАТУРЫ)
Аннотация
Целью настоящей публикации является попытка систематизации современных данных, основанных на принципах доказательной медицины (контролируемые рандомизированные исследования, систематические обзоры, метаанализы) в области диагностики и терапии неспецифической поясничной боли.
Показано, что неспецифическая поясничная боль является преобладающим видом поясничного болевого синдрома. Основой диагностики неспецифической поясничной боли должны являться подробный сбор анамнеза и тщательное физикальное и неврологическое обследование.
Значение визуализирующих диагностических методов ограничено пациентами с наличием “красных флагов”. Как в случае острой, так и хронической неспецифической поясничной боли основой терапии с хорошо доказанной эффективностью являются обучение пациента, манипулятивные техники и специальная лечебная физкультура. В случае хронической неспецифической поясничной боли высокой эффективностью обладает поведенческая терапия. Доказанной эффективностью в отношении болевого синдрома и функционирования пациента обладают нестероидные противовоспалительные средства. В случае хронической неспецифической поясничной боли дополнительно могут быть назначены мышечные релаксанты и антидепрессанты.
